Please note following points:

1. Please print and submit claim form in Landscape format.

2. Mobile claims and other claims are as per our claim policy.

3. The claims are mentioned in claims form, should be attached corresponding documents.

4. In case of mobile claim, should be attached cash receipt. (not need the bill copy)

5. Claim form must be signed by claimant.

6. Without signature and incomplete forms are rejecting.

7. All forms are must be reach before 7 th of every month. After I cant process the claims.
